The first step in treating your veins is to schedule a comprehensive consultation and examination with Dr. Raffy Karamanoukian, a board certified vein surgeon and a diplomate of the American Board of Phlebology (Vein Surgery).  Dr. Karamanoukian is a fellow of American College of Surgeons, a diplomate of the American Board of Phlebology, a diplomate of the American Board of Plastic and Reconstructive Surgery, and an Active Member of the American College of Phlebology.


Our office provides comprehensive treatments for all types of cosmetic spider veins and varicose veins.  Our treatments include endovenous laser treatment, endovenous RF vein treatments, sclerotherapy, vein injections, ultrasound sclerotherapy, transdermal laser, and microphlebectomy.  Your initial consultation will begin with a full examination and vein mapping in which Dr. Karamanoukian will evaluate your legs and thighs and determine the presence of unhealthy veins underneath the skin and below the skin.  This is performed with a combination of transilluminated vein mapping as well as ultrasound vein mapping. 


By examining the areas and determining the root cause of your vein clusters, Dr. Karamanoukian can advise you on a specific regimen that focuses on comprehensive and regimented treatment of your vein problems.  Not all cosmetic spider veins can be treated with transdermal laser and injection and that is why our comprehensive approach begins with a complete examination to determine the extent of underlying vein problems in the development of spider veins.  This reduces recurrence rates and increases the yield of a spider vein treatment.  Our consultations are followed by an ultrasound examination to determine the function of your veins as well as the degree of venous insufficiency and venous reflux found within the deeper vein network.  By diagnosing unhealthy veins in your legs, the root cause of your vein problems can be more readily understood. 


Risk factors for vein disease are many, including genetic predisposition, advanced age, female sex, weight gain, pregnancies and childbirth, oral contraceptives, hormone replacement, occupational standing or sitting, decreased aerobic activity, smoking, blood clots, trauma, flat feet, hernia surgery, cardiovascular disease, constipation, and chronic lifting of heavy weights.

Endovenous laser treatment is a minimally invasive approach that has effectively replaced old fashioned vein stripping and vein surgery.  The target for endovenous laser treatment includes the unhealthy veins that are associated with venous reflux.  By addressing these veins with laser energy, the leaking and unhealthy vein valves in your legs can be eliminated and the circulatory system and improved.  The end result of successful endovenous laser treatment is ablation of the unhealthy vein valves in your thighs and overall improvement in venous reflux.

The main treatment associated with endovenous laser treatment includes EVLT, which is a state of the art technique that requires minimal recovery and is performed under local anesthesia with the patient awake and alert. CLARIVEIN: The Clarivein endovenous vein treatment is ideal for patients who have an active lifestyle and have little time for recovery or downtime.  Clarivein represents an important innovation in endovenous treatment as it does not require perivenous anesthesia.  This is mostly because the Clarivein system doesn not utilize thermal coagulation as a way to treat the unhealthy veins in your legs and thighs.

There are many options for sclerotherapy or injection treatment of your veins.  Dr. Karamanoukian is proficient in a number of sclerotherapy options including foam sclerotherapy, dynamic foam sclerotherapy, ultrasound sclerotherapy, transilluminated sclerotherapy, and micro sclerotherapy.  These injection treatments are less painful and less risk than old fashioned saline injections performed in previous years.  The patients are pleased to eradicate their unwanted spider veins and thread veins while at the same time noticing improvements in the appearance of their veins.  Sclerotherapy can be performed on an outpatient basis with no anesthesia and minimal recovery.  In some cases, Dr. Karamanoukian will advise you to wear compression stockings in the period after your sclerotherapy and may require you to pretreat your skin to prevent any hyperpigmentation of the skin overlying your thread veins.

Micro vein surgery still remains a viable option for patients who have bulging varicose veins that uncomfortable and painful.  These bulging varicose veins often have a dense network of connecting veins and are likely set by multiple channels.  The treatment of these bulging veins begins with a comprehensive vein mapping and ultrasound to determine the root cause of the bulging veins.  If the network is identified, Dr. Karamanoukian may recommend a procedure known as microphlebectomy in order to safely remove and ligate these unwanted and unhealthy veins with local anesthesia.




One of the most important components in vein treatment is proper diagnosis.  Your primary care physician or internist may recommend a diagnostic test known as an ultrasound.  The best thing about an ultrasound is that it is painless, but it provides a lot of useful information about your veins.  Ultrasound is a body imaging system that allows physicians to see two things: 1) What your deeper veins look like underneath the skin and 2) How the blood flows within those healthy and unhealthy veins.  The diagnostic ultrasound allows for pretreatment mapping of your veins and helps diagnose a condition known as venous reflux.   


Our office employs ultrasound to 'map out' varicose vein networks in the pre and intraoperative setting.  In addition, Dr. Karamanoukian is an expert in ultrasound guided sclerotherapy to obliterate unhealthy veins underneath the skin. 


BEFORE YOUR VEIN ULTRASOUND: Ultrasound tests do not require any preparation from your part.  We ask you to drink plenty of water prior to your examination and to go to the bathroom, if needed, prior to the ultrasound. 


DURING YOUR VEIN ULTRASOUND: Your vein ultrasound is not painful.  Focus on understanding the vein mapping and ask questions about your veins and the findings.  Dr. Karamanoukian will be happy to discuss his findings during and after the ultrasound procedure. We have found it important to have a trained physician perform the ultrasound in most cases, as the diagnostic value of the test can be incorporated in your treatment plan and surgery.  During the vein ultrasound, a transducer will be applied to the skin with some gel and an image of your veins will appear.


AFTER YOUR VEIN ULTRASOUND: There is no aftercare that is required after your vein ultrasound. Dr. Raffy Karamanoukian will use this opportunity to go over your vein treatment plan with you.  If you have venous reflux and measurable symptomatic venous insufficiency, then a procedure known as endovenous ablation may be recommended for you.  Endovenous ablation can be performed with the EVLT, VNUS, Venefit, or Clarivein procedures.  Often, treatment of venous reflux and venous insufficiency can help solve problems associated with your veins, including pain, discomfort, throbbing, cramping, leg swelling, itching, burning, and restless legs.

Varicose veins can occur on any part of the legs and thighs, but often appear in characteristic patterns on certain parts of the legs.  Varicose veins represent bulging abnormal veins of the superficial venous system.  As a result of venous insufficiency and venous reflux, an inadequate return of blood causes veins to reflux and bulge underneath the skin.  Varicose veins that occur near the inner thigh are very commonly associated with a Boyd’s perforator vein or as a result of incompetent greater saphenous veins.  Choosing the right treatment for bulging varicose veins begins with a comprehensive consultation with board certified vein surgeon Dr. Raffy Karamanoukian.

The choice of varicose vein treatment depends on the severity of venous problems as well as the severity of associated vein symptoms.  The patients who have varicose veins often have symptoms of pain, discomfort, leg tenderness, muscle fatigue, cramping, throbbing, restless legs, and skin changes.  The patients may also have venous ulcers and venous eczema which may represent poor venous blood flow in the medial and lateral ankles.  The patients who have skin changes associated with darkening of the skin along the lower legs may also benefit from endovenous treatment.  Although vein stripping is generally considered an older type of vein treatment, there is a meaningful need for micro vein stripping in some patients who have large bulging varicose veins.
