What is an alar base reduction during rhinoplasty?

An alar base reduction or nostril narrowing surgery is performed to narrow the width of the nose at the nostril or nasal base.  This is usually performed in combination with a rhinoplasty procedure in order to provide a more esthetic proportion to the base of the nose in comparison to the overall height and length of the dorsum.  In some cases nostril reduction surgery, or alar base reduction can be performed alone in order to narrow the nasal base.  During surgery a wedge shape piece of the nostril base is removed and then closed using intermediate layered sutures. 

How do I know if I need an alar base reduction during rhinoplasty?

The base of the nostrils and nose are known as the nostril sills.  In patients who have wide nasal base a procedure known as an alar wedge or alar base reduction is performed to narrow the base of the nose.  This adds proportion to nose in patients who have a wide nasal base in comparison to the height and projection of the nasal dorsum.  During rhinoplasty procedure, Dr. Raffy Karamanoukian will remove a wedge shaped portion of the nostril base and suture to repair the area in order to reduce the width of the nose.  This can be done in combination with a rhinoplasty or as an isolated procedure.
